PATIENT RIGHTS & RESPONSIBILITIES
You have a right to the following:
- Service - reasonable access to health services offered according to Student Health & Counseling Services (SHCS) guidelines and a clear explanation of SHCS policies.
- Choice of Service Providers - you may choose or change health professionals from the pool of providers at the SHCS.
- Dignity and Respect as an individual regardless of your gender, race, ethnicity, national origin, culture, sexual orientation, class, physical ability, creed, lifestyle or other human attributes.
- Privacy concerning discussions, examinations and treatments regarding your care.
- Confidential treatment of all communications and records relating to you (release of information in your medical record requires your written consent), except as required by law.
- Participate in decisions involving your care.
- Know and understand the assessment of your problem, what tests are being done and why, what treatment / medicine is recommended and its side effects and risks, the prospects for resolutions of your problem, and how this problem can be prevented in the future.
- A second medical opinion before making any decision and the right not to be treated, but you should be made aware of the medical consequences of refusal.
- Be informed of Fees incurred during your visit and of SHCS payment policies.
- Know and understand how to express Grievances and Suggestions to the organization. See the Front Desk, call 510-885-3735 or e-mail shcs@csueastbay.edu for the “Complaint Form”.
You are responsible for the following:
- Be honest and direct about everything that relates to your needs for services.
- Cooperate with all persons involved in the care process and respect the privacy and rights of other students, treating all students and staff with respect.
- Keep appointments on time or notify the Student Health & Counseling Services if you cannot do so.
- Make sure you understand your problems or needs and ask regarding the diagnosis, treatment plan, test, policy, or anything else that concerns you.
- Follow the prescribed treatment plan or advise your provider if you are unable or do not want to, and discuss and negotiate an alternative plan.
- Report changes or adverse effects to your provider.
- Answer questions fully to present a complete picture of your illness or complaint (may involve answering personal questions which are relevant and important to your problem).
- Know your professionals by remembering names of the people who serve you.
- Inform your provider of any advanced directives, living wills, or medical power of attorney that could affect care.
- Arrange transportation to and from the SHCS, including the cost of ambulance to another facility.
- Pay fees on the day of your service or make payment arrangements with the Student Health & Counseling Services Cashier.
- Read and understand all documents and instructions given or sent to you by SHCS staff, including your Horizon e-mail and secure messages, and ask a SHCS staff member if you have any questions.
- Be respectful toward all health care professionals and staff, as well as other patients and visitors.
